Your customer rents the equipment before the patient ever owns it. Medicare's definition of durable medical equipment requires that it withstand repeated use, serve a medical purpose and suit use in the home (42 CFR Part 414, Subpart D). Items classified after January 1, 2012 must also last at least three years. The same subpart puts a whole class of it on capped rental: monthly payments for up to 13 months of continuous use, then title transfers to the patient. So an HME supplier buying beds from you is buying rental fleet. It judges you on how long a unit lasts, not only on the invoice.
The supplier standards turn into requests on your desk. To hold Medicare billing privileges, a DME supplier certifies that it meets 30 standards in 42 CFR 424.57. They include accreditation for the specific products and services it bills. It may fill orders by contracting with other companies, but must produce contracts or other documentation on request. It must maintain and repair or replace the items it rents, and honor all warranties. Each of those reaches your team as a request for an invoice, a warranty record or a part.
The serial number is where service lives. A rented power chair needs batteries, joysticks and casters over its life, and the supplier owes the repair. The technician standing next to it knows the model and the serial number, and rarely your part number. Warranty status decides who pays. If a unit fails mid-rental, the replacement has to be the same or an equivalent make and model, which means your catalog has to keep that model findable.
Complex rehab is configured to one person, then it waits. A complex rehab power chair is specified around one user's seating and positioning. It is also the one class of power wheelchair where Medicare still requires suppliers to offer purchase at first delivery. Dozens of power mobility device codes, starting with K0856 and K0861, sit on CMS's required prior authorization list. The manual wheelchair base K0005 and a pressure reducing support surface join it nationwide on October 28, 2026. Your configured price has to hold while that decision comes back.
Every account buys on different terms. An HME supplier buys rental fleet. A skilled nursing facility buys beds it will own. A home health agency, which Medicare's definition names alongside suppliers as a furnisher of DME, buys around its caseload. Each carries its own contract price and minimums. The DMEPOS competitive bidding program has sat in a temporary gap since January 1, 2024, and CMS says it will conduct bidding for Round 2028. Your price will be part of every bidding supplier's math.
